Custom ReGlen Implant for Complex Glenoid Reconstruction
Patient Overview:
The patient presented with a history of multiple prior shoulder surgeries. Following a traumatic event, the existing shoulder implant failed, resulting in severe glenoid bone loss.

Due to the extent of bone deficiency, standard off-the-shelf revision solutions available in the market were not suitable for achieving stable fixation or anatomical reconstruction. Considering the complexity of the defect, a patient-specific custom ReGlen implant was recommended to reconstruct the glenoid and enable revision of shoulder arthroplasty.
Surgical Planning:
In close collaboration with the operating surgeon, the Jajal Medical 3D design team carried out a detailed digital preoperative planning workflow to ensure surgical accuracy and implant predictability.
CT-based 3D segmentation of the scapula and residual glenoid anatomy was performed.

The extent of bone loss and available bone stock were carefully analyzed to determine optimal implant geometry and fixation strategy.
A custom ReGlen implant was designed to anatomically reconstruct the deficient glenoid.
The fixation strategy was planned based on zones of maximum bone purchase to ensure long-term stability.
Custom ReGlen Implant Design and Manufacturing:
The patient-specific ReGlen implant was designed and manufactured to restore glenoid anatomy and provide a stable base for revision to reverse shoulder arthroplasty.
Key Features of the Custom ReGlen Implant:
Custom-fit geometry matching the patient’s native glenoid anatomy.
Porous lattice structure at the bone, contact regions, and over the central peg to promote osseointegration and long-term biological fixation.
The central peg design is to provide primary stability and accurate implant positioning.

Locking screw configuration strategically oriented toward areas of maximum bone stock for enhanced fixation strength.

Designed to be compatible with standard glenosphere components available in the market.
Patient-Specific Surgical Guides (Jigs):
To ensure accurate intraoperative execution, dedicated patient-specific guides were developed as part of the solution, making preoperative planning smooth and efficient.

A Glenoid Axis Guide was designed to assist in the precise creation of central holes for accurate peg placement.
A separate locking screw guide was designed to ensure accurate pilot hole positioning and screw trajectory, minimizing intraoperative guesswork and improving fixation reliability.
Post-op Outcome:
The custom ReGlen implant was successfully implanted, achieving stable fixation and accurate reconstruction of the glenoid anatomy. The custom ReGlen implant fit precisely as planned, allowing for a secure attachment of the glenosphere and restoration of joint mechanics.

Early postoperative assessment demonstrated stable implant positioning, improved structural support, and favorable conditions for bone integration.
Clinical Impact:
This case highlights the critical role of custom 3D-printed shoulder implants in managing complex revision scenarios with severe glenoid bone loss where conventional implants are inadequate.
Through advanced imaging, patient-specific design, and precision manufacturing, Jajal Medical delivered a personalized ReGlen solution that restored anatomical integrity, ensured implant stability, and expanded the surgical options available for complex shoulder reconstruction, reinforcing the value of custom implants in next-generation orthopedic care.
